I use Suitcase Fusion from Extensis. You can download a free 30 day demo at http://www.extensis.com
It allows you to activate or deactivate fonts as needed and automatically works "on the fly" with most Adobe applications.
There are also several other good font managers out there and others may chime in as to what they use.
By all means, you should use a font manager. It will allow you to easily organize your fonts and it will also help your computer run faster by not overtaxing the Windows font folder.
BTW, does your forum name happen to be a business name?

Currently in the digital version of "a pile". I've just got them all dumped in a folder named, oddly enough, FONTS. It's currently over 1 GB....but there are duplicates of duplicates in there plus zip files from copying font files from obsolete computers over the years.
I use Adobe Font Folio or The Font Thing to add or remove fonts. I've never worried with turning fonts on or off. I've had as many as 700 installed at one time and never noticed font lag from it. Currently there are about 75 installed...and honestly 95% of what I do uses 10 of them.

Font Book
I am a Mac user and have used Suitcase and Linotype FontExplorer for years but with the release of 10.6 Font Book does a good job with all of my basic fonts that I use all the time and leave all my fonts on a DVD and when I need a certain one just copy over.

I have all my fonts in two different sorting schemes. The first and most used is alphabetically by style. The second is alphabetically by manufacturer. This allows me to browse my fonts quickly no matter how I am looking for it.
I can, for example, look at all my script fonts in one showing or I can look at all the fonts I have from any given manufacturer in one showing. I use a font manager named TypoGraf which can handle viewing, installing and moving to a different directory (how I got them sorted in the first place).
