Summa T series S2 140 plotter screwing up a little too much lately

I am not sure why it is happening but this plotter sometimes just fucks up all the cuts. I watch it and I see it reading all the registration marks from Flexi and then I walk away and let it do its thing... So today besides ruining some expensive print and cut graphics I ran 156 rectangular cut decals about 2.5" x 6" The first sheet cut flawlessly and then I loaded the second sheet which is identical and ran that one. Outside of the first row of cuts, everything after that is way out by over 1/8" or more and progressively got worse the farther into the cutting it went. I am not sure how I can circumvent this issue.
I sometimes wonder if the rollers are not holding down strong enough but I don't even think there is any adjustments for that...very frustrating and costly.
This plotter ran flawlessy for a few years and now it seems almost every day I am running into issues. any ideas on what may be happening?

I ran two more cut jobs from the same files as the screw up and they cut flawlessly...I think maybe sometimes I don't leave enough vinyl on the back end or front end and it gets confused.

I get that from time to time too. Most of the time, the vinyl was loaded at an angle or there was a wrinkle between the rollers. The black registration marks also need to be printed perfectly sharp, and the Summa should be properly calibrated. That makes it work much better. Also, when reading the registration marks, set it as slow as possible instead of using that high-speed feature.
