1/4" White Router Cut Acrylic Vendor/ Supplier

I'm looking for a vendor or referral for a vendor to supply a Router Cut 1/4" White acrylic. With a Digital Print in a Solid Pantone of Green with the business name in white mounted to it, Size is apx. 20x30 (I will be field verifying the wall dimensions and location.) see attached for the shape. the grey text below will be cut vinyl.
