Acuity 350GT head failure

We have had good luck with this machine except for a few issues.
One issue that seem to be too coinsidental is that twice just after ( next day ) the years maintainence is performed under contract we get at least one or two head failures. this year fuji forgot to do the service and things were great. Then the filter change etc and true as every one full and one partial both black. too much.

Here is an added tidbit,

Had the Tech change the black heads over to the white etc and guess what , problem did not migrate to the white. SO Heads are good.!!!!
Turns out tech had changed black ink bag during the maintenance routine and its defective.
