Anyone here make house plaques?

I'm in need of a dimensional bronze-looking house plaque for a wedding present. I need it before September 16, the big day. I can live with a standard shape but I want to design the content. The plaques available on line []have pleasing shapes but the content is limited to a couple of lines of text with a couple of lines of boilerplate. I'm looking for something that looks like it fell off of a historical monument.
