Anyone recognize this plaque?

This a bit of a long shot but I thought I would try.

These plaques were outsourced by my company before my time and the paperwork is nowhere to be found.

The Superintended has changed at the school and I need to find a replacement insert.

The Plaque is 8" x 5.75" and the name slot is 7" x 2".

The site is about 2 hours away and any money I make would be eaten up in a site visit.

You can try Clark Systems.. they will also custom make..

Have used them a lot with a previous company and they are always good to deal with and decent priced
