Bill of lading for vehicles

Anyone please direct me to where i can buy a vehicle inspection template for a 3 part bill of lading including the liability disclosure on the back. Please and thank you

We've been printing these alot lately. Seems like the vehicle transport business is hopping in my area. I just scanned a clean copy in that I was given and made the changes on the pdf.

You could also go to your nearest truck stop and look at buying a small package of these to scan and use.
