custom / odd sized take-out bags

i have a potential customer who has been searching for to-go bags that fit her 12x12 by 2" tall pastry boxes. she needs the footprint to be 12x12 (or slightly larger) and the height can be as small as possible.

the bags can be either plastic or paper.

i've called a few different places, but they haven't gotten back to me with an answer.

have you used anyone that can be recommended?
