Yeah, writing out a company name in a font does not a logo make.

In my opinion, your client needs a primer on logos and branding before this goes any further.

He needs to understand how and why and look beyond what his "vision" is to focus on what will work better for him as a symbol of his business to promote his brand.

He's a plastic surgeon, right? Approach it on that level. When a client of his comes in, and says "I want that nose!" does he just do it, or does he consult them on what would be more aesthetically pleasing and serve them better?

You're the professional here, sometimes that means pushing back a bit, but in a way that you care about the work you're doing for the client and want to give him the best you have to offer.
