Need Help Mutoh XPJ-661uf White Text Peeling

Hello everyone,
At my company we have 3 mutoh xpj-661uf printers, we use VerteLith and Illustrator on the software side and UH21 inks. Towards the start of the year (late January) we did a job with 2 types of sign; the first was blue frosted acrylic with white test and braille - the other was a cream with blue text and clear braille.

As of today about 80% of the blue panels have had their print fail and are peeling off of the panels - all of the cream panels are completely intact still and some of these are in the same locations as the blue panels.

We believe that the issue is in our white ink but any insight would be really appreciated

It was a material issue if the cream panels survived. If ink is bad, it’s bad 24/7.

Do you do any type of prep or primer prior to printing?
 
A bit more info,
The panels used are back painted frosted - so the face where we print is just frosted acrylic. We use Deco Color plastic primer although we have just learnt that this doesn't thermal cycle well and as most of our print jobs fly out from the UK to the US the primer is doing nothing and the material can be expanding whilst in the air.
The print is failing on over 300 panels and this was printed over 2 printers so could be a bad batch of ink?
We are doing some testing with hot and cold to see if that is causing the issue but to be honest I don't really know why it's failing.

We tried adhesion promotors, etc. for years and the best solution we have found is to just use acrylic optimized for printing when printing ADA with UV. Plaskolite Optics DA non-glare or Acrylite Digital print pop-touch.

It sounds like the print is a first surface on P95 material. We have had problems with the 1/4" Plaskolite DA, it has speckles throughout the material, although the DA material does take a second surface print very well.
