I have had good luck with F&M Expressions when needed. They only do transfers, not the entire garment, and I believe they are out of NJ?
But if you've got a heat press in house, the transfers will work fine.
Another one that I prefer for transfers is Dowling Graphics, but they are out of Florida.

A company named NBM owns Printwear magazine and you can find suppliers in any state in that magazine. Also you'll find that most any screenprinter will give you contract rates if you give them enough business annually. Finally, art for apparel is separated differently so expect some art charges.
