Shift in the blue what causes that?

So a friend of mine has done this print that failed. It's just a simple print with white text on top of a blue box and it came out horrible.
Was it the printer (HP 26500, the rip(Onyx), the person who made a mistake, or something else?
I have never seen this before or later :eek:

If you have a spare cyan printhead, I'd start there. Alternatively you can turn up the passes, it looks like ink starvation, the printhead just finally catches up when it has the little bit of downtime going over the white and not firing.
