Need Help Source for blank plaques

Anyone know of a source for the oval blanks for this plaque? It's small, 3½" x 4½" x ⅜". The plate with the raised number and braille is separate and stuck on top. We just need to find a source for the base plaque. We can do custom routing, but a manufactured piece might be cheaper if it's available.
