Just a heads up you are using tables which is fine but the better way is to use CSS and divs on your page as most spiders do not read or crawl tables very well.
in fact for what you posted all you need are labels and text areas inside the form attribute.
Here is a snip of code from a form...
I just pasted your code to a new page and uploaded it to my servers and in google chrome and Internet explorer 9 it looks right.
I am not sure why you are having a problem.
You could change the background color and see if that works for you here is the code.
Style="background-color...
I use dreamweaver post the code and I will see if I can help.
What the others said should work.
Post the code.
Are you using Java, php, css in a separate sheet or is it all html?
Home based is great if you have the drive to do it right.
You have to show up to work on time.
You need to treat it as if it was a storefront based business.
You need to charge prices that are fair in your market dont undercut really low just because you have a lower overhead...
I know a rep of a supplier who says that 8518 is more optically clear than Oracals 290 optically clear laminate. I have not verified this but if 8518 is shifting the color on your print then its not optically clear (or at least not to the standards that we want).
Well its really not that hard you need to install the splitters into the ink lines. Then put new ink lines in(or you can flush the lines but best to replace them). I hope you have splitters if not you can get a hold of Mimaki there pretty friendly.
The splitters are just little T's with brass...
We dont run Mutoh printers but the whole wave technology doesnt band thing is not true it sounds to me like you have banding in your print. You still get banding its just in a wave pattern instead of straight.
Again I dont know anything about your printer but try adjusting your media comp...
As posted before I would use the quick dry epoxy if the temp is real cold you co uld help it out by using a heat gun to keep it warm around the epoxy.
If you have problems you could use some VHB tape or home depot equivelant to hold the sign inplace while the epoxy dries.

Some laminates are known to have a blueish tint to them and will cause the color shift you are seeing.
Try another brand or use an optically clear laminate.
You dont have to remove the head to flush it out with a syringe on the JV33. Just run the head over to the left side and open the cover then remove the dampners put a catch container (I like to use a alcohol bottle cut open to fit) under the head then hook up syringe and push the flush through...
if your really worried about it I would make sure i have full ink carts in the printer and change the sleep cleaning to every 4 to 6 hours it will waste a lot of ink but you should be safe.
I sue to let my JV3 go for 3 or 4 days when i first got it many years ago and had the sleep clean set...
