Couple of things that could be happening. I don't have an Epson so not sure with that particular model, but inkjet printers in general all have similar issues.
1. The head(s) could just be going bad. The older they get the more I've noticed they can print a perfect nozzle but not sustain in printing. Since they're firing on a nozzle test, it means they're not totally blocked.
2. Ink delivery issue. The pumps aren't working as they should and are not pumping enough ink into the reservoir or pumping air bubbles which causes nozzles to drop out.
3. Low & High pressure issue. On my flatbeds, I have two air pumps, one that attempts to pull air out of the ink lines, and another that holds the ink right at the firing nozzle so it doesn't drop out nor does it get sucked up back into the reservoir. If the Low Vaccum is too low, ink just falls out of the heads. If it's too high, it pulls the ink back up into the reservoir and then your nozzles don't have ink to fire and end up firing air causing drop out.
