Laminated adhesive vinyl. It will work great for a "presentation" puck, but for actual match use nothing will hold up!
I would order these from a jobber (around $10 plus shipping for a page of twelve 2.5" x 2.5" stickers, much less if bought in quantity).

Hockey pucks are screen printed with a solvent based ink. Be careful as there are pucks coming from Czech Republic, Slovakia, China, Russia etc Some have migrations of oils to the surface causing yellowing. We supply the official puck supplier to the NHL.
May be best just to order them from Inglasco already decorated as that is their business, day in and day out.
