Adjust your expectations and realize that as an employer, you are going to have to train people. Find people's strong points and try to utilize them and cut the cord quickly for those that you feel are not trainable. You can't teach people how to think per se. I'm not contradicting myself...

As already said, an approval will not trump a mistake or get you out of fixing it no matter who signed it. I don't think that was ever the intention of them either.
why not just do a head test on the roll you have loaded close to the leading edge? You will probably cost yourself more in time swapping rolls to save a buck on material
