Looks like you may need to change out the black/cyan head. Lot of banding in those boxes. An overall maintenance check from a technician may prove useful
Def looks like the cable is the prime suspect. Try reseating them and check to make sure they are in working order. Make sure main power is turned off before doing this.
Agreed. Send it back and get it replaced. Thousands and Thousands of cartridges sold daily it's prob just a bad sensor. Dont spend too much time on it.

A few thoughts on this...
-Is the machine completely 100% level?
-Does it happen with all materials? Specific materials?
-Are you using the proper heat settings? Media Clamps?
-When you attach the material to the take up are you attaching it in the center or the sides?
