Look for a printer who has a Gerber Edge. Should be an easy job without any "screens setup" and the quality will be better and more consistent than a flatbed.
It is printed by a Gerber Edge (reverse print) with thermal foils onto clear polycarbonate. We do these all the time for generators, etc to protect them from scratches.
Do you need the actual glass/polycarb panel itself, or just the "graphic"?
The rollers have to be outside the registration marks.
The head needs positioned in front (about 1-2 inches) of the black box.
When you load/clamp the material your "start point" for that operation has to be well in front of this area.

Unfortunately I do not have this exact template. Our installers here do those by hand trimming or in this case would just put down knifeless tape before installation.
Use a PVO template and give it about 2" bleed and 1-2" inset all around and it'll be gravy.
With the newer "subscription" updates from Adobe, I find a lot of them to be pretty flakey. I would see if there is an update you haven't updated to yet and go ahead with it. That usually fixes the issue here if a reboot can't cure it.
Photo of the gray adhesive isn't the best, but many of the adhesives look the same, so you can't really tell them apart by sight since there are SO many of them.
I've never seen black adhesive before and we use a decent amount of 3M.
