Knowing what size it will be really helps. You could easily get a custom shaped cabinet with a trimcap face for that size pretty inexpensively. Then just decorate the face with a single printed decal on translucent media.

What about something like this? Powdercoated aluminum panface backer, ACM secondary panel and some dimensional letters. These are a bit deep because of the boldness of the typeface, but you could use a nice classy font like Optima, Trajan, etc. and give it a little depth, not not knock people...
I would suggest that you consider creating two separate cabinets to help on fabrication and maintenance costs.
The icon section seems to dominate over the name section.
If these are simple backlit cabinet components, I don't see where adding dimensional elements to them will really enhance...

We lightly sand the edges of the letters and Scotchbrite the faces and then fog coat the first coat. You do have to wait a day to do the second coat, but it covers pretty well and levels really nicely. It creates a really nice hard finish that doesn't need any topcoat.
