Looks like you're doing multiples of the same file, are you doing them all as a single file together? If so, try doing them as single files (print copies with reg marks around each one individually instead of marks around a bunch of them), that way you're just cutting each as a 5.5x38 file...

A lot of things can cause that... Too low of a print resolution selected, wrong print profile for the media, old media. If it's not a vector image and the image resolution is low they'll print like that too.

You just stated the issue right there. All those codes are limit sensors, head lock, carriage connection, encoder... Plus the noise are all things that would happen when the belt is missing teeth and allows play and slop in the carriage movement. Replace the belt.

As long as it's not from a head strike, soaking the head might bring it back. That's where I would start.
