Are you using genuine Roland cap tops or knock offs? There's a huge price difference but the savings isn't worth the aggravation. Change the springs every so often too. I put a piece of white vinyl on the metal behind the print head carriage so I could see better to do the cap height adjustment.

Be sure the power is OFF before tinkering with the flat, ribbon cables. If you pull one out with the power on, it'll destroy the printed circuit board on top of the print carriage and you'll need to replace it.

Black ink (likely) has the greatest amount of pigment in it and thus the black print head gets 'tortured' more than the other heads. It doesn't appear that any of your nozzles are plugged (missing in the test print) but many are getting deflected spray and that's why your prints are fuzzy...

Replace the cap top with a QUALITY (ie expensive) unit. A little extra expense is not worth the aggravation and down time. Also change the springs if you haven't done so in some time. Edit: if the cap top is leaking, ink won't be drawn into the damper as well. And this is how cap tops fail...
