Controller failure...
I haven't seen this in the field about two years myself, but it is a most likely a failure of the controller (below)... stuck solenoid. I replaced several of these over the years...

Swollen Yellow Captop
Sorry to hear of your mess!
Most likely the yellow captop on your machine (JV3?) became swollen enough to contact the printhead which would allow the yellow ink to be wicked through the system.... from cartridge bay to the waste bottle.

Zip tie
In these cases I use a small zip tie as a retainer... simply snip it off if you need to pull out the axle for whatever reason.
I see this at least once or twice a week.

Broken blade
VP series has a maximum downforce of 300 grams. Not sure where your 400 comes from...
In any case it's probably a broken blade... or possibly the thumbscrew holding in the bladeholder was not tightened (sounds silly but I see it).

Dating issue
Looks like a timing/dating issue.
Turn off the printer and unplug it from the wall for a minute. Plug it in and turn it back on.
I would bet that this will cure the problem.
On a side note, it looks like your black head is shot.

Vertical is off
Vertical alignment is way off on the black... this is a physical adjustment.
I always tell people to not let their tech leave before running a small print job...
especially after a head adjustment/alignment. Just in case stuff like this happens.

VersaCAMM SP-300
I have a couple of Roland VersaCAMMs that I harvest for parts. Mainboards are already removed and ready-to-ship (mainboards are blown).

Remove the battery
Please remove the battery and see what happens (lithium battery is on the main board).
Battery only maintains time and date. All of your settings are safe.

Using MT profiles on non-MT jobs
I wonder if you may be using MT profiles on non-MT jobs.
In VersaWorks there are MT specific profiles with [MT] in the title. For example Generic Vinyl 2 [MT]
Only use [MT] MARKED profiles for MT jobs. Otherwise you will purge MT each time you print... even if...

If you are doing small stuff like decals and heat transfers, then I would go with a CMYKLcLmCM setup. The results will be outstanding for graphics that will be viewed up close.

Env Match?
Sounds like a it could be as simple as an Environmental Match issue.
Menu > ENV MATCH
Env Match should be set everytime you a planning on running print/cut. Machines prior to VP/Pro III need to set this manually through the menu.
Machines VP/Pro III and later do this automatically...
Linear Rail?
In my 7+ years of servicing Roland printers, I have only seen happen this once in the field.
Turned out to be the linear rail being out of alignment... just enough to cause an electrical to chassis rub during the scan and a resulting short to ground. This turns off the machine...
